I didn't leave my sexual preference blank because I don't know, it's just a lot more difficult to explain than 5 or so choices will allow.
I am a young gay woman (still over 18 =P) raised an effeminate straight boy, but despite being definite about my female mind, I have no plan of going through reassignment or hormones.
